Composition

Glucosamine, chondroitin sulfate and hyaluronate in one syringe.

A sterile, viscoelastic solution for intra-articular injection containing N-acetylglucosamine (30 mg/ml), chondroitin sodium sulfate (30 mg/ml) and sodium hyaluronate (16 mg/ml, 3 MDa) in a 2.25 ml pre-filled glass syringe. The hyaluronate and chondroitin sulfate are physically associated through hydrogen bonding; the product is not chemically or irradiation cross-linked.1

The three ingredients

Glucosamine, then chondroitin sulfate, then hyaluronic acid.

Chondroitin sulfate and hyaluronate are glycosaminoglycans (GAGs); N-acetylglucosamine is a precursor in their synthesis. Hyaluronic acid is the lubricating component. The descriptions below are the manufacturer’s.1

All three are components of the extracellular matrix of articular cartilage and synovial fluid, alongside collagen. The manufacturer describes glucosamine as essential for the formation of hyaluronic acid, chondroitin sulfate and keratan sulfate, and positions the formulation as supplementing the synovial fluid with components of that matrix.1

Glucosamine

30 mg/ml (3%) · 67.5 mg per syringe

N-acetylglucosamine is a precursor in glycosaminoglycan synthesis. The manufacturer describes it as reducing cartilage degeneration and supporting regenerative processes, and as a structural component of articular cartilage and synovial fluid.1

Chondroitin sulfate

30 mg/ml (3%) · 67.5 mg per syringe

A sulfated glycosaminoglycan. The manufacturer describes it as protecting cartilage and reducing inflammatory activity in the osteoarthritic joint, and as the natural cross-linking agent for the hyaluronate chains, bound in a 1:1 ratio.1

Sodium hyaluronate, 3 MDa

Hyaluronic acid

16 mg/ml (1.6%) · 36 mg per syringe

High-molecular-weight (3 MDa) sodium hyaluronate, not chemically or irradiation cross-linked. The manufacturer describes physical self-association with chondroitin sulfate through hydrogen bonds and hydrophobic interactions, forming a stable complex in aqueous solution.1

Rheology

Elastic and viscous moduli, as reported.

G′ is the elastic (storage) modulus, the solid-like part of the response, and G″ is the viscous (loss) modulus, the fluid-like part; the brochure reports both at 0.5 Hz, which it labels at rest, and 2.5 Hz, which it labels running.1

Physical cross-linking

Associated, not chemically cross-linked.

The manufacturer describes the hyaluronate and chondroitin sulfate as physically associated through hydrogen bonds and hydrophobic interactions, forming a complex in solution. No chemical or irradiation cross-linking is used.1

The manufacturer states that this association gives the solution stability and resistance to degradation by hyaluronidase, and that the absence of chemical or irradiation cross-linking is intended to reduce reactions after administration.1

Indication

Knee osteoarthritis, mild to severe.

The manufacturer’s brochure states the indication as: ‘As a viscoelastic supplement or as a replacement for the synovial fluid in the knee joint’ and ‘For the symptomatic treatment of mild to severe knee osteoarthritis’.1

  • As a viscoelastic supplement or as a replacement for the synovial fluid in the knee joint.1

  • For the symptomatic treatment of mild to severe knee osteoarthritis.1

Intended effects

As described by the manufacturer.

The manufacturer describes the intended effects as pain relief, lubrication and mechanical support, and protection and regeneration of the cartilage. These are the manufacturer’s statements; individual results vary.1

Administration and schedule

One syringe per treatment, twice a year.

TriCombo is for intra-articular injection by a physician. One syringe per treatment. The manufacturer recommends two treatment cycles per year, at six-month intervals.1

The brochure does not specify a technique beyond intra-articular injection. Approach and any use of image guidance are the treating clinician’s decision.1

Contraindications and warnings

The instructions for use govern.

This website does not summarise contraindications. Current instructions for use remain authoritative.2

  • Contraindications, warnings and precautions are listed in the instructions for use supplied with each pack.2

  • For intra-articular injection only, by a physician, as stated on the manufacturer’s packaging.1

  • Single-use pre-filled syringe; do not re-sterilise or reuse.2

Presentation and storage

A single-use, pre-filled glass syringe.

Supplied as a single-use, pre-filled 2.25 ml glass syringe (Becton Dickinson), sterile, for intra-articular injection.1

Storage conditions and shelf life are specified in the instructions for use and on the pack label.2
